ADMISSION PROCEDURE
For direct Admission, please present at the Admission section your:
- Attending Physician’s admitting order
- Government issued Identification card (to ensure that correct data is encoded in the system)
- Philhealth Requirements (if PBEF portal says NO)
- Card and Letter of Authority ( for HMO members)
Patient and/or relatives can choose from the following room categories depending on the Availability. Patients that need isolation must occupy private rooms. You may choose from the following room types:
- Ward
- Semi-Private Room
- Small Private Room
- Deluxe Room
- Standard Suite
- Premiere Suite
Cash payments have to be made directly to our Cashiers, who are the ONLY authorized personnel to accept payments. For HMO members, Letter of Authorization have to be secured, approved and submitted to the Business Office staff prior Admission.
Our Business Office can be reached at 553-8185 local 120, 117 and 102.
For security and safety reasons, a maximum of 2 companions are allowed to stay overnight with the admitted patients occupying a private room while 1 companion only is allowed to stay overnight for patients occupying ward rooms. A letter of authorization from the guardian/s of children below 7 years old is required when patients/relatives want them to be at the hospital premises during admission. Guest room for patient’s relatives maybe available depending on the census and availability and this may be coordinated with the Admitting office.
Our visiting hours are from 9:00am to 9:00pm
During your stay, only drugs and supplies from our Hospital Pharmacy and Central Supplies will be administered. Drugs and supplies bought outside the hospital is not allowed.
